Job Summary

The Care Manager is responsible for supporting the Program Director in the Treatment Foster Care program. This position supports the recruitment of therapeutic foster parents and coordinates the interdisciplinary, trauma-informed plan of care for therapeutic foster parents recruited from inquiry to completion. Additionally, this position conducts parent education with clients, including adoptive, foster, and/or kinship caregivers and acts as a liaison between client/family, the Program Director, and/or peer mentor to ensure quality support is provided promptly and effectively.


Duties & Responsibilities

  • Completes intakes and assessments of new clients
  • Enters and maintains client contact and demographic data in CMS
  • Provides resources/professional referrals specific to needs of clients
  • Conducts in-home and telehealth parent education clients
  • Acts as a liaison for Treatment Foster Care Agency and Chosen
  • Provides parent orientation sessions for potential Treatment Foster Care families
  • Provides Chosen’s model of care to Treatment Foster care families in-home and via telehealth
  • Conducts inter-disciplinary meetings to ensure continuum of care
  • Completes other duties as assigned.

Education & Abilities:

  • Bachelor’s Degree in Social Work or similar field with 2-4 years related experience
  • Master’s Degree in Social Work preferred
  • Licensed in the State of Texas strongly preferred
  • Trust-Based Relational Intervention® Practitioner strongly preferred
  • Solid working knowledge of current industry norms and challenges
  • Demonstrated experience in client support and direct client communications
  • Technically competent with Microsoft Office suite
  • Demonstrated experience eliciting cooperation from clients
  • Strong interpersonal skills: display behavior that will enhance the client experience
  • Experience proofreading and/or editing program materials
  • Bilingual skills strongly preferred

Requirements:

  • Prolonged periods of sitting at a desk and working on a computer.
  • Must be able to lift up to 20 pounds at times.
  • Quiet workspace and high-speed internet and phone connection to support telehealth services and video conferencing.
