The Position

The Care Manager delivers Chosen’s model to the family system with therapeutic interventions as needed. Working directly with children and their families, this position performs various in-person trauma-responsive modalities. This position is in-person and works under the supervision of the Program Manager in Florida to ensure holistic healing and care.

 


 

Job Responsibilities

  • Process client applications
  • Enter and maintain client contact and demographic data in client management system
  • Complete clinical assessments as needed
  • Create actionable plan for the client family system
  • Conduct trauma-informed parent coaching through in-person or telehealth, as applicable
  • Coordinate with interdisciplinary team to meet needs of the family
  • Attends supervision with Programs team
  • Completes documentation in accordance with the SOP
  • Provides crisis support/management as needed
  • Other tasks as assigned

 


 

Qualifications

  • Bachelor’s or master’s degree in social work with 2-4 years related experience and/or training, or equivalent combination of education and experience
  • License in social work or mental health strongly preferred
  • Trust-Based Relational Intervention® Practitioner strongly preferred
  • Trauma-Informed training or certification
  • Solid working knowledge of current industry norms and challenges
  • Demonstrated experience in client support and direct client communications Technically competent with Microsoft Office suite
  • Quiet workspace and high-speed internet and phone connection to support telehealth services and video conferencing
